ATHLETIC DEPARTMENT STUDENT-ATHLETE REQUIRED FORMS

Lynchburg College Student-Athletes and Parents:

The Lynchburg College Athletic Department and the NCAA require each student-athlete to complete particular forms in order to participate on an athletic team.  All forms are mandatory and are kept on file in the Athletic Director's office. 

The required NCAA and Lynchburg College forms are distributed at the first team meeting.  It is the responsibility of the student-athlete to complete the required forms completely and correctly.  Forms that are not completed correctly will be returned to the student-athlete.  It is important that you read and understand each form before signing.  Please Note:  Parent/Guardian signature is required if the Student-Athlete is a minor.  Forms will be mailed home along with a self-addressed stamp envelope.

Other general information:

NCAA - General Amateurism and Eligibility Form for International and Select Student-Athletes - Division III

International student-athletes are required to contact the Director of Athletics for the appropriate forms.

NCAA - Permission to Contact:  Self-Release - Division III

The purpose of this form is to grant Division III student-athletes permission to contact other Division III institution's athletics department staff about a possible transfer.  This form is ONLY to be completed if you wish to transfer to another institution.  This form is not returned to the head coach or Lynchburg College Athletic Office.
